FDA Approves First Oral PCSK9 Inhibitor, Lipfendra, for High Cholesterol Management

A pivotal shift for patients with hypercholesterolaemia

In a significant milestone for lipid management, the U.S. Food and Drug Administration (FDA) has approved Lipfendra or enlicitide as the first oral proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor authorized in the United States. The approval marks a pivotal shift for patients with hypercholesterolaemia, particularly those with heterozygous familial hypercholesterolemia (HeFH), who often struggle to reach target cholesterol levels with conventional therapies.

PCSK9 protein: a driver of cardiovascular disease

A simplified functional role of the Low-Density Lipoprotein Receptor (LDLR) is analogous to "garbage trucks" of the body, as their job is to patrol the bloodstream, grab "bad" cholesterol (LDL-C), and for cellular processing.

Under normal conditions, the body regulates these "trucks" using a protein called PCSK9. When PCSK9 binds to the LDLR, it signals the cells to destroy the receptor. High levels of PCSK9 mean fewer "trucks" are available to clear cholesterol, leading to the high blood levels that drive atherosclerosis and heart disease.

Using advanced mRNA display technology, researchers were able to synthesize a peptide that mimics the binding precision of an antibody while being small enough to be formulated for oral delivery.

The Mechanism of Action: Blocking the "Destruction Signal"

Lipfendra is designed to intervene in this specific interaction. Unlike traditional small-molecule drugs that might try to mimic a chemical reaction, Lipfendra acts as a barrier to prevent PCSK9 binding LDLR.

The peptide is engineered to bind with high affinity (Ki = 5 pM) to the LDLR-binding domain of the PCSK9 protein. By physically occupying this site, Lipfendra prevents PCSK9 from binding to LDLR and significantly increases the body's ability to clear LDL-C from the blood.

Because these types of peptides do not easily pass through cell membranes, the study utilized permeation enhancers (such as sodium caprate) to facilitate "paracellular" absorption—essentially helping the drug slip between the cells of the intestinal lining.

A New Frontier in Lipid Therapy

While PCSK9 inhibitors have long been recognized for their potent ability to lower low-density lipoprotein cholesterol (LDL-C), they have previously been available only as injectable monoclonal antibodies (mAbs) or small interfering RNAs. These injectable therapies, such as Repatha (evolocumab) and Praluent (alirocumab), have demonstrated high efficacy but may face barriers to widespread adoption due to the requirement for injections.

Lipfendra, a small-molecule macrocyclic peptide, offers a transition to an oral, once-daily dosing regimen. By binding to plasma PCSK9, enlicitide prevents the protein from interacting with the LDL receptor, thereby preventing the degradation of these receptors and increasing their presence on the surface of hepatocytes to clear LDL-C from the blood.

Clinical Evidence: The CORALreef HeFH Trial

The FDA clearance is supported by robust clinical data, most notably from the Phase 3 CORALreef HeFH trial. The study evaluated the efficacy and safety of enlicitide in adults with HeFH who were already receiving stable lipid-lowering therapy, including high- or moderate-intensity statins.

Safety and Administration

The safety profile of enlicitide was reported as favorable, with no significant difference in adverse events (AEs) or serious adverse events (SAEs) compared to the placebo group. The most common side effects were mild, including nasopharyngitis, influenza, headache, and nausea.

Administration: Patients must take the medication on an empty stomach in the morning, waiting at least 30 minutes before consuming food or beverages.

Market Impact and Competition

The approval of Lipfendra is expected to significantly disrupt the PCSK9 market. Analysts note that Merck & Co. is employing an "aggressive, volume-driven pricing strategy" to encourage adoption by primary care physicians.

About the Study

The CORALreef HeFH trial was a multi-country, double-blind, placebo-controlled study involving 303 participants across 59 sites in 17 countries. Further research is ongoing in the CORALreef Lipids trial to evaluate the drug's effects in patients with atherosclerotic cardiovascular disease (ASCVD).
